Abstract
A prospective study was carried out to evaluate the sexual status of 87 patients who had undergone prostatectomy(TUR:71, suprapubic prostatectomy: 7 , retropubic prostatectomy:9) for benign prostatic hypertrophy from march 1991 to January 1992.
All
patients were interviewed personally about sexual history by the same physician before and 6 months after surgery. Their ages ranged from 50 to 85 years, with a mean of 66.6(¡¾7) years.
Seventy four patients(85%) were potent(fully potent:70, partially potent:4) at the time of operation. Five fully potent men(5.7%) became totally impotent and 17 fully potent men(19.5%) became partially potent. Of patients who were potent
preoperatively,
22 patients(30%) experienced a great deterioration in their sexual status and 40 patients(4.6%) reported improvement in potency after operation. Retrograde ejaculation was noticed in 41 patients(47%). In univariate analysis of various risk
factors,
great deterioration was related to younger age and drinking coffee and that of decreased satisfaction was related to their chronic illness, decreased stamina, smoking and low socioeconomic status. Postoperative change in voiding symptoms was
related to
decreased satisfaction to some extent but it was not statistically significant.
Our results indicate that prostatectomy might induce a deleterious influence on sexual status and sexually active male should be warned of the risks of impotence and decreased satisfaction.
